Fishbone Diagram

The diagram of cause and effect, also known as Fishbone Diagram, is meant to identify, explore and project all possible causes of a problem. It is the organized method to represent the relation of the \"effect\" to all possibilities of \"cause,\" by dividing them into sets of causes.

By using a group of people or facts, you can raise all causes that bring about the effect being analyzed. Thus, each effect may have a great number of causes. Once causes are raised, action plans are grouped and created.

A detailed diagram of cause and effect will have the form of a fishbone; that is why it is also called fishbone diagram. From a list of possible causes, the most likely causes are identified and selected for an in-depth analysis.

 

Procedures

To register a fishbone diagram:

1. In Registers menu, click Fishbone Diagram.
2. Click New.
3. Enter:
Name: Enter a single name for the fishbone diagram.
Scorecard: Select the scorecard to which it will belong.
Indicator: Select the indicator to be analyzed.
Description: Enter a description for the fishbone diagram .
4. Click Save.

The fishbone diagram was created, but it does not have causes or effects registered.

5. Click Analysis folder.
6. Click Edit.
7. Click New/ Effect.
8. Enter:
Description: description of the effect.
9. Click the Effect registered.
10. Click New/Cause.
11. Enter:
Description: description of the cause.
12. Click Save.

After the registration, the fishbone diagram created will be available in the register.
